Sounds like you might have the Healing Brush Tool or Clone Stamp Tool selected instead of the Spot Healing Brush Tool.
(click and hold on the icons in the toolbox to see the hidden tools)
You don't need to Alt/Option click to set a source point for the Spot Healing Brush Tool.
Just click and move the brush over the areas you want to heal.
